When Giuliana Rancic, long-time host of E! News, first saw her new son, she said it was “the best single moment of my life.” Recently, on the eve of Duke's first birthday, the TV personality and reality star spoke to Dear Doctor magazine about her growing family, her battle with cancer — and the importance of starting her child off with good oral health.
“Duke will have his first visit with the dentist very soon, and since he is still a baby, we will make his visit as comfortable as possible,” Giuliana said. That's a good thought — as is the timing of her son's office visit. Her husband Bill (co-star of the couple's Style Network show) agrees. “I think the earlier you can start the checkups, the better,” he said.
The American Academy of Pediatric Dentistry concurs. In order to prevent dental problems, the AAPD states, your child should see a dentist when the first tooth appears, or no later than his or her first birthday. But since a child will lose the primary (baby) teeth anyway, is this visit really so important?
“Baby” Teeth Have a Vital Role
An age one dental visit is very important because primary teeth have several important roles: Kids rely on them for proper nutrition and speech, and don't usually begin losing them until around age 6. And since they aren't completely gone until around age 12, kids will depend on those “baby teeth” through much of childhood. Plus, they serve as guides for the proper position of the permanent teeth, and are vital to their health. That's why it's so important to care for them properly.
One major goal for the age one dental visit is to identify potential dental issues and prevent them from becoming serious problems. For example, your child will be examined for early signs of dental diseases, including baby bottle tooth decay which is a major cause of early childhood caries. Controlling these problems early can help youngsters start on the road to a lifetime of good oral health.
Besides screening your child for a number of other dental conditions or developmental problems, and assessing his or her risk for cavities, the age one visit also gives you the opportunity to ask any questions you may have about dental health in these early years. Plus, you can learn the best techniques for effectively cleaning baby's mouth and maintaining peak oral hygiene.
Breezing Through the Age-One Visit
To ease your child's way through his or her first dental visit, it helps if you're calm yourself. Try to relax, allow plenty of time, and bring along lots of activities — some favorite toys, games or stuffed animals will add to everyone's comfort level. A healthy snack, drink, and spare diapers (of course) won't go unappreciated.
“We'll probably bring some toys and snacks as reinforcements,” said Giuliana of her son's upcoming visit. So take a tip from the Rancics: The age one dental visit is a great way to start your child off right.

If you follow the hit TV reality show Amazing Race, you know that professional-hockey-playing brothers Bates and Anthony Battaglia won the $1 million prize in the latest globe-spanning competition. You may also have witnessed Anthony removing his false front teeth from time to time — like when he had to dive for pearls in Bora Bora. Since he plans to resume his sports career, Anthony wears a partial denture to fill the gap in his classic “hockey mouth.” He has said that when he finally hangs up his skates, he will use some of his Amazing Race prize money to get new, permanent teeth. When it's time to get that new smile, Anthony, like many people, will have to choose between two good options for permanent tooth replacement.
The preferred option for most people is dental implants. In this system, tiny titanium posts substitute for the root part of your missing tooth (or teeth). These are placed beneath your gum line in a minor surgical procedure we perform right here at the dental office. The amazing thing about dental implants is that they actually fuse to your jawbone, allowing your replacement teeth to last a lifetime.
The titanium implant itself is not visible in the mouth; the part of an implant tooth that you see is the lifelike crown. Virtually indistinguishable from your natural teeth, the crown is attached to the implant above the gum line. Dental implants can be used to replace a single tooth, multiple teeth, or even all your teeth. You don't necessarily need one implant for every tooth because implants can support bridgework or even a complete set of prosthetic teeth.
The second-best option is a natural-tooth fixed bridge. In this system, we use healthy natural teeth on either side of the empty space left by a missing tooth (or teeth) as supports for one or more of the prosthetic teeth that will fill the gap. The downside is that in order to turn these healthy teeth into supports (which are referred to in dentistry as “abutments”), we need to remove some enamel and then cap them. This procedure can leave those teeth more prone to decay than they were before. But with regular dental exams and good oral hygiene on your part, bridgework can last many years.

Olivia Newton-John, now in her early 60's, is still a fresh-faced picture of health — with a radiant smile to match. How does she do it? She does it with healthy habits learned from her German-born mother, Irene.
“I love greens, and as many organic vegetables as possible,” Olivia recently told Dear Doctor magazine. “From spinach to salads to beets — pretty much any and all greens!”
Olivia credits her mom with instilling her lifelong love of healthy foods. Irene used dark bread rather than white bread for sandwiches and even made her own yogurt — which she used as a topping on baked fruit for dessert.
“Growing up, my mum really taught us some great eating habits,” Olivia told the magazine. “When I was a girl in school, all of my friends would have cakes and cookies and fun foods but my mum was all about teaching us to eat healthy foods and to be very aware of what we were putting into our bodies. At the time I was annoyed about it, but looking back now I thank her for teaching me at an early age to eat healthily.”
Irene paid particular attention to her children's oral health. “My mum always made us brush and floss after every meal so, once again, like the foods we ate, she taught us early about the importance of great dental hygiene,” said Olivia, who has an older brother and sister.
As a mom herself, Olivia passed those healthy habits down to her daughter, Chloe.
“I always insisted on regular dental checkups and limited sugar, especially in soft drinks — they were never in our fridge,” she said.
Parents do play an important role in developing healthy oral habits from the very beginning, starting with proper tooth-brushing techniques. By age 2, a brushing routine should be established using a smear of fluoride toothpaste. For older toddlers, parents can use a child's size soft toothbrush with water and a pea-sized amount of fluoride toothpaste. Children need help brushing until at least age 6, when they can generally take over brushing by themselves and also learn to floss.
The point of a good daily oral hygiene routine is to remove the film of bacteria that collects daily along the gum line, and in the nooks and crannies of teeth. Effective daily removal of this biofilm will do more to prevent tooth decay and promote lifelong dental health than anything else.

It might seem that adults who play aggressive, high-contact professional sports (ice hockey, for example) have the highest chance of sustaining dental injuries. But for many — like NHL hall-of-famer Mike Bossy — their first injured teeth came long before they hit the big time.
“The earliest [dental injury] I remember is when I was around 12,” the former New York Islanders forward recently told an interviewer with the Huffington Post. That came from a stick to Bossy's mouth, and resulted in a chipped front tooth. “Unfortunately, money was not abundant back in those days, and I believe I finally had it repaired when I was 16.” he said.
You may also think there's a greater chance of sustaining dental trauma from “collision sports” like football and hockey — but statistics tell a different story. In fact, according to the Academy of General Dentistry (AGD), you (or your children) are more likely to have teeth damaged while playing soccer than football — and basketball players have a risk that's 15 times higher than football players.
So — whether the game is hockey, basketball or something else — should you let the chance of dental injury stop you or your children from playing the sports they love? We think not... but you should be aware of the things you can do to prevent injury, and the treatment options that are available if it happens.
Probably the single most effective means of preventing sports-related dental injuries is to get a good, custom-made mouth guard — and wear it. The AGD says mouthguards prevent some 200,000 such injuries every year. And the American Dental Association says that athletes who don't wear mouthguards are 60 times more likely to sustain harm to the teeth than those who do.
Many studies have shown that having a custom-fitted mouthguard prepared in a dental office offers far greater protection then an off-the-shelf “small-medium-large” type, or even the so-called “boil and bite” variety. But even if you do have an injury, don't panic: Modern dentistry offers plenty of ways to repair it! The most common sports-related dental injuries typically involve chipped or cracked teeth. In many cases, these can be repaired by bonding with tooth-colored composite resins. For mild to moderate injury, this method of restoration can produce a restoration that's practically invisible. It's also a relatively uncomplicated and inexpensive procedure, which makes it ideal for growing kids, who may elect to have a more permanent restoration done later.

Singer Olivia Newton-John's daughter Chloe is now a lovely, grown woman, but Olivia recently recounted to Dear Doctor magazine a rather creative method she found to sooth Chloe's teething troubles many years ago.
“When Chloe was a baby and teething I remember using a frozen bagel for her sore gums,” Olivia said. “She loved it!”
Cold is often very soothing to a teething child's gums. In fact, the American Academy of Pediatric Dentistry (AAPD) recommends using a clean, chilled, rubber teething ring, or cold wet washcloth. Chilled pacifiers can also be helpful. Be sure not to freeze teething rings or pacifiers as ice can actually burn sensitive mouth tissues.
Older teethers can sometimes find relieve from cold foods such as popsicles (or bagels!) but make sure your child eats these sugar-containing foods only at mealtimes so as not to promote tooth decay.
If your baby has not yet begun the teething (or tooth-eruption) process, you can expect it to begin usually between six and nine months. It may, however, start as early as three months or as late as twelve months.
Teething symptoms vary among children, as does the length of time it takes for a tooth to make its appearance. But many parents notice the following signs:
- Biting and gnawing
- Gum swelling
- Chin (facial) rash
- Disrupted sleeping patterns
- Ear rubbing
- Decreased appetite
These symptoms are usually most bothersome during the week that the tooth is breaking (erupting) through the gums, starting about four days before and lasting about three days after the tooth appears.
Occasionally, teething discomfort can be considerable. If that is the case with your baby, you can give her or him acetaminophen or ibuprofen in the appropriate dose (check with your pharmacist if you're not sure what that is). The medicine should be swallowed — not massaged into the gums, as this can also burn. Numbing agents should not be used for children under 2, except under the advice and supervision of a healthcare professional.
